Undiagnosed Lameness?

Just a guess here, but I would suspect something neurological. If you have tried everything you listed there are nothing is working the only thing I can think of is some sort of long term meningitis or EPM. Has the vet done anything to test for that? Does she display any ataxia along with the soreness? The uneven gait may be signs of this.
